Family/Personal
Records and Vital Info
EMT workers want you to know what they need if called
to your home in an emergency, and your family needs to know their medical
history and where important financial papers and wills are.
You can keep all of this stored near the phone in a 3
ring binder, along with addresses and phone numbers for your family for an
emergency. Teaching your
children where to find the notebook and who to share that information with
can save your life.
Create a sheet for each of the 4 categories below:
Ambulance/Hospital/Doctors
You should have a sheet for every member of the
family, headed with the person’s name and social security number.
This SS number is crucial to Emergency workers who will be looking
for your name in the medical records database in your town, and there may
be 2 or 3 people with your name.
If they can’t positively identify you, they may
confuse you with someone else, or never realize that you’re allergic to
something they might be giving you.
