Important records your family needs on hand

 

Family/Personal Records and Vital Info

EMT workers want you to know what they need if called to your home in an emergency, and your family needs to know their medical history and where important financial papers and wills are.

You can keep all of this stored near the phone in a 3 ring binder, along with addresses and phone numbers for your family for an emergency.  Teaching your children where to find the notebook and who to share that information with can save your life. 

Create a sheet for each of the 4 categories below: 

Ambulance/Hospital/Doctors 

You should have a sheet for every member of the family, headed with the person’s name and social security number.  This SS number is crucial to Emergency workers who will be looking for your name in the medical records database in your town, and there may be 2 or 3 people with your name.  

If they can’t positively identify you, they may confuse you with someone else, or never realize that you’re allergic to something they might be giving you. 

  • Name, social security number
  • Address and telephone number
  • Date of birth
  • Doctor’s names and numbers, and last time you saw that doctor
  • Medications you are on
  • Past medical problems you have had, and any that you are aware of now
  • Childhood diseases each person has had, (measles, mumps, chickenpox, etc.)
  • Blood type if known
  • Allergies, especially to medications
  • Name your preference of hospitals or doctors
  • State your religious preference
  • Signed release for emergency medical treatment for your children in case you are absent

Important Papers/ Legal Documents Sheet 

  • Location of wills, trusts, stocks and bonds, cemetery plot deeds, house deeds, car titles, life insurance policies, house and car insurance policies, safe deposit box keys, funeral instructions, living will instructions, passport, certificates of immunization, birth, death and marriage certificates, diplomas or graduation certificates
  • Name and contact info for Executor of will or estate
  • Name and contact info of Legal Guardian or person holding Power of Attorney
  • Names and contact info of relatives
  • Your lawyer, your funeral home, your pharmacy
  • Banks, account numbers, life insurance on account?, safe deposit boxes?
  • School records, Shot records, Pet records, rabies shot info,
  • Adoption papers, genealogical reports
  • Electric, Water, Gas and Garbage companies, account numbers and info for emergencies
  • Location of power or fuse boxes in your home, emergency shut offs for water and gas etc.
  • List of current bills, amounts, balances and contact info for them, along with due dates

Emergency contact sheet 

  • Baby sitters
  • Family member’s cell phone numbers
  • Extended family who don’t live with you, contact info and how they are related to you
  • Neighbors’ names and numbers
  • Christmas card list
  • Electric, Water, Gas and Garbage companies, account numbers and info for emergencies
  • Location of power or fuse boxes in your home, emergency shut offs for water and gas etc.
  • Numbers for Fire and Police, Ambulance and other emergency numbers
  • Food delivery services that you usually use
  • Mechanic/garage/gas station/towing service that you usually use
  • Insurance company, policy and coverage limits

What to do in an emergency instructions 

  • For babysitters, children and others, make a list of instructions for various catastrophes
  • List location of each item to gather or use, such as candles and matches.
  • Bad Weather Kits for cars and home (include blankets, flares, water, food, matches, candles, hurricane lamps for home use only)
  • Tornado, hurricane or other weather disaster (blankets, clothes, water, food, matches, candles, medicines) List where to go if these events are announced, and how to contact each other if separated in a disaster.  Have a location decided upon and an out of town relative as a contact go between.
  • Violent attack, break in, robbery, kidnapping (location of weapons, baseball bats, hiding places etc.)
  • Fire, escape plan
